The single application is submitted through the portal of the Central Registry in electronic form, and the applicant of the single application is provided with access to the information system.

The applicant of the single application is obliged to register on the portal of the Central Registry, that is, to confirm his identity by using an electronic certificate issued by an authorized certification body, in accordance with the law.

When submitting a single application, evidence is submitted, or confirmed, on the basis of which the property of the insured person is determined, and the type of evidence depends on the basis of the insurance. The list of evidence on the basis of which the status of the insured person, i.e. the insured person, changes in insurance and termination of the status of the insured person is determined, is prescribed by the Regulation on the content, form and method of submitting a single application for mandatory social insurance, uniform methodological principles and a uniform code of codes for data entry in The single database of the Central Register of Mandatory Social Insurance (hereinafter: Regulation) and implemented in the form of electronic application.

On behalf of the applicant of the single application, the application can be submitted by a natural or legal person who possesses a qualified electronic certificate, based on the authority to submit the application.

Exceptionally, for a natural person who does not have the technical ability to submit an application in electronic form, in accordance with the provisions of Article 15. of the Act on the Central Register of Compulsory Social Insurance, and based on the attached evidence, a single application is submitted by:

1) an authorized official of the organization for mandatory social insurance, who has a qualified electronic certificate on the portal;

2) an authorized official of the Republic Health Insurance Fund through a modified application.

In the process of submitting a single application, upon acceptance of the registration of the applicant of the single application, a check is made as to whether:

  • the applicant of the unified application is registered as a person authorized to submit the unified application;
  • the application contains all the data required for the registration of the insured and insured persons;
  • are the data contained in the single application, and refer to the general data on the insured, the insured person and the person liable to pay contributions, identical to the data found in the Single Database.

If, in the process of submitting a single application, it is determined that the data on the insured, the insured person and the person liable to pay contributions – a natural person, are not in the Single Database, personal data on the insured and the insured person shall be entered by the applicant based on the evidence at his disposal.

If in the process of submitting the application it is determined that the conditions for its acceptance have not been met, the applicant of the unified application is informed of the reasons for not accepting the unified application.

The applicant of the unified application can remove the reasons for the non-acceptance of the unified application during the procedure of submitting the unified application or subsequently in the repeated procedure of submitting the application.

Upon acceptance of the application and registration in its database, the Central Registry issues a certificate of completed application – Form M-A, the outer form of which is printed with the Regulation and forms an integral part of it. Also, the accepted application is automatically forwarded to all mandatory social security organizations.

The confirmation, in addition to the data from the single application, also contains the date and time of submission of the single application, the number under which the single application was filed, a list of confirmed evidence, as well as special statements of the insured, for whom the application is submitted, and the applicant’s agreement with the data provided by the authorized officer of the organization for mandatory social insurance entered in the application and it is delivered to the applicant of the single application at the electronic address indicated on the single application.

If the person obliged to submit the application is not also the applicant of the single application, the applicant is obliged to immediately deliver to the person obliged to submit the application a certificate of completed application – Form M-A.

The person obliged to submit the application is obliged to deliver a copy of the confirmation of the completed application – Form M-A, to the insured, that is, to his employee, within three days.
